Due to changes in our practice and staff, we have made changes to our pricing structure outlined in your new Fee For Service Agreement effective 1/1/2024. We have updated that form and the Office Policies Form to be re-signed under the Questionnaires tab. This new structure will allow us to best serve our patients.
