Due to changes in our practice and staff, we have made changes to our Office Policies and Procedures.. We have updated that form to be re-signed under the Questionnaires tab. We have also started using Heidi AI and there is a now a form to be signed asl well. This new structure will allow us to best serve our patients. Please let us know if you have any questions.
